This data includes information from all ESRD facilities, including Facility Name, CMS Certification Number, Alternate CCN1 and 2, Address 1 and 2, City name for the facility State that it is located in Zip Code associated with the facility Network it is part of data on Hemoglobin gt 12 g/dL Performance Rate (percentage of patients with hemoglobin greater than 12 g/dL) Hemoglobin gt 12 g/dL Measure Score as well as a National Average Hemoglobin gt 12 g/dL Measure Score to benchmark against. How to use the dataset

This dataset contains performance rates of US ESRD facilities for anemia in 2016. The data includes information on facility name, CMS certification number, alternate certification numbers, address, city, state, zip code, Latent Network affiliation as well as the performance rate and measure scores with respect to Hemoglobin greater than 12 g/dL.

To make use of this dataset for analysis purposes you can filter the data by selecting a particular facility or group of facilities based on their name or location information. You can also look at individual facility performance by looking at the Hemoglobin greater than 12 g/dL Performance Rate and Measure Score columns alongside other relevant medical measure scores from different areas (ex.
